I'm sorry to say that it hasn't been entirely squashed. =(

The GOOD news is, that it no longer rears its ugly head when you attempt to make something. It now only shows up when you fail to make something. I did a lot of experimenting at the alchemy table to confirm this.

I know that when you attempt to craft something the script first checks your inventory to see if you have the required items. This is where the bug used to show up. When you succeed, it simply takes those items from your inventory and all is well. Should you fail, however. I think another decision tree is accessed where it decides randomly if you loose all the ingredients, or only some of them. Somewhere in this part of the script, the TMI bug is cropping up again.

I hope this helps you find it and smash it for good. In the mean time, at least we can still craft, and we don't get that annoying double ingredient loss like we used to when the TMI bug popped up.
